New Employee Documents

Congratulations on your employment with the McGregor Independent School District.

It is important that you complete the forms listed below and accumulate the required documents to establish a personnel file.  We will be unable to issue your first paycheck if your file is incomplete.

Please call the Administration Office (254.840.2828) as soon as possible to schedule a time to complete the hiring process.  We will meet at the MISD Administration building at 525 Bluebonnet Parkway, McGregor, Texas. 

Please complete and print the forms below and bring them with you to our meeting:

•  Personal Data Report
•  W-4 Form
•  I-9 Form (Federal Employment Eligibility Verification)
•  Statement concerning Job not Covered by Social Security
•  Direct Deposit Form
•  New Hire Reporting Form
•  Retire / Rehire Questionnaire
•  Worker's Compensation Acknowledgement
  Affidavit of Applicant Offered Employment

You will also need to gather the following documents and bring them with you:

•  Original transcript of all college hours
•  Teacher Service Record
(if you have prior experience in a school district); You may use the following form to request these records from your previous district:  Request for Records

Finally, please bring the following identification documents:

•  For payroll purposes: Driver License or US Passport AND Social Security Card
•  For I-9 Form:  bring the documents for the employer to review under Section 2.  see the "List of Acceptable Documents" columns A, B, or C.
